Primary Care

Insurance and cash-pay options are available. Coverage depends on your individual insurance plan, benefits, and confirmed network participation.

Men's Health, Women's Health, Medical Weight Loss & Spravato® Treatment

These services are currently cash-pay.

Lab Services

Lab billing depends on the service associated with your care. Lab work connected with Primary Care may be billed through insurance where applicable, while lab work associated with a cash-pay service follows that service’s payment arrangement.

What happens after I submit my insurance information?

Submitting your insurance information allows the Clayton Health & Wellness team to carry out insurance verification before your visit.

Verification helps us understand your coverage, but it is not a guarantee of payment by your insurance company. Deductibles, copays, coinsurance, exclusions, prior authorization requirements, and other patient responsibilities are determined by your individual health plan.
